We are a 501(c),(3) organization.

We accompany you or meet you at your doctor's appointments  so that you will have a better understanding of your care plan and the management of your chronic illness.  

We also help with filling out applications for SNAP, HEAP, and WIC programs. We assess for Social Determinants of Health (SDoH) and make appropriate referrals based on the results of the assessment.

The best part about our services is that we can help explain the treatment or procedure you need and reduce the risk of misunderstanding the advice or diagnosis of your doctor.
